Something like this. It is not on English but you can see the form. I want to have space to write my name, address, phone... And below my basic information I want to have contact form. In contact form I need four boxes for name, e-mail, subject of message and message.

You can use contact component of default Joomla. In our SJ News template, we're using it too.
